Product Recalls for the Week of July 18, 2016:

  • Kid’s Insulated Water Bottles: GSI Outdoors, Inc. is recalling five styles of kids’ insulated water bottles with printed graphics. The lead solder at the exterior base of the bottle contains high levels of lead. Lead is toxic if ingested by young children and can cause adverse health issues. The bottles are 13.5 ounces capacity with the following color prints: Dino Bones, Flower Power, Orange Grid camo, Purple Tie Dye Butterfly and Robo Shark. The item identification number 297684 is printed on a sticker on the bottom of the bottle. Also printed on the sticker are “PO#844” and “BB2D2-LLB-R45-0413.” The bottles were sold exclusively at L.L.Bean retail stores, online at www.llbean.com and in L.L.Bean catalogues from July 2015 through May 2016. Consumers should immediately stop using the recalled water bottles and contact L.L.Bean for a full refund. Recall Date: 07/19/2016
  • Things Remembered Children’s Jewelry: Things Remembered, Inc. is recalling silver heart bracelets and sweet charm necklaces for children because the clear surface coating on the bracelets and necklaces contain lead in excess of the allowable limit. Lead is toxic if ingested by young children and can cause adverse health issues. The 6-inch, silver-plated bracelets have 10 silver hearts and circle links, a lobster claw clasp, and one silver heart-shaped charm. The necklaces are gold plated on a 13-inch box chain with a 2-inch extender, a lobster claw closure and three charms – a pink ice cream cone, a pink crystal cupcake and a gold-plated tag with “SWEET” engraved. The items were Sold exclusively at Things Remembered stores nationwide and online at www.thingsremembered.com from February 2015 through June 2016. Consumers should immediately take the recalled jewelry away from children and contact Things Remembered for a full refund, including shipping charges, or a replacement gift of equal value.   • Diocto Liquid: PharmaTech LLC, the manufacturer of the Rugby® – branded product, is voluntarily recalling all lots within the expiry of Diocto Liquid, a docusate sodium solution due to a risk of product contamination with Burkholderia cepacia. Use of docusate sodium liquid contaminated with B. cepacia may result in serious infections that could be life-threatening in patients with compromised immune systems and in patients with chronic lung conditions such as cystic fibrosis. All lots with NDC 0536-0590-85 are included in the recall. Diocto Liquid was distributed nationwide to wholesale and retail facilities including hospitals and pharmacies.  PharmaTech is notifying its distributors and customers by recall letter and is arranging for return of all recalled products. Consumers, pharmacies, and healthcare facilities that have product which is being recalled should stop using and dispensing them immediately. Recall Date: 07/19/2016
  • Tippin’s Key Lime Pie: Tippin’s Gourmet Pies, LLC is voluntarily recalling select Key Lime Pies because they may contain flour with undeclared peanut residue. Tippin’s initiated the recall after we became aware that our supplier, Kellogg Company, was conducting a recall of graham cracker crumbs because they may contain undeclared peanut residue. The graham cracker crumbs were used to make the crust for 8 inch Key Lime Pies with the UPC code 680816050057; lot numbers 04-26-16, 04-27-16, 05-03-16, 05-04-16, 05-17-16, and 05-31-16. As an update to the June 21, 2016 recall announcement, Tippin’s is expanding the recall to include lot number 05-31-16. Additionally, lot numbers 04-21-16, 04-25-16, 05-05-16, and 05-11-16 are not affected by this recall. This product was sold to distributers throughout Kansas, Missouri, Texas, Oklahoma, Arizona, and Illinois. In the event that consumers believe they have purchased the listed product and have an allergy concern, they should return the product to the store they purchased it from for a full refund, or discard the product.   • Salted Almond 70% Dark Chocolate: Theo Chocolate is voluntarily recalling 3 ounce Salted Almond 70% Dark Chocolate bars because they may contain undeclared milk. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume this product. The Theo Chocolate Salted Almond 70% Dark Chocolate bars have the affected lot 3037431 and the expiration date 05/2017 and they are found on the back side of the bar Theo Chocolate Salted Almond 70% Dark Chocolate bars are wrapped in a white paper envelope, weighs 3 oz., UPC is 8 74492 00170 4, and stored at room temperature. Salted Almond 70% Dark Chocolate was distributed to retail stores in AZ, CA, CO, GA, IA, ID, FL, MI, MT, ND, NH, OR, NJ, NY, PA, TX, WA, and WI. Consumers who have an allergy or severe sensitivity to milk are urged not to consume this product and should return it to the place of purchase for a full refund. Recall Date: 07/18/2016
  • No-Salt Added Black Beans: Faribault Foods, Inc. is initiating a voluntary recall of a limited number of 15 ounce No-Salt-Added Black Beans due to the potential presence of plastic and/or metal pieces within the product. This voluntary recall covers one specific production code and was distributed under the following labels: Hyvee No-Salt-Added Black Beans, La Preferida Low Sodium Black Beans (Frijoles Negros Bajo En Sodio), Our Family No-Salt-Added Black Beans, Sprouts No-Salt-Added Black Beans, Mrs. Grimes No-Salt-Added Black Beans, Kuner’s No-Salt-Added Black Beans (Frijoles Negros), HEB Black No-Salt-Added Beans (Frijol Negro Sin Sal). The voluntary recall is limited to the production code of 15 ounce product manufactured at the facility located in Faribault, MN with can code “Best By 04 APR 2019 258F2 095 16 XXXX.” To locate the production code, consumers should look at the bottom of the can. No other production codes, sizes or brands of Faribault Foods, Inc. products are affected by this recall. Consumers who may have purchased the products listed above should not consume them but should instead return them to the store where purchased for a refund or replacement. Recall Date: 07/15/2016
  • P.F. Chang’s Home Menu Brand Meals: ConAgra Foods is expanding a voluntary recall of P.F. Chang’s Home Menu Brand products due to the potential presence of small metal fragments (2-9mm) in the sugar used in the sauce. This voluntary action is being undertaken because of impacted sugar from a supplier. An initial recall was issued on July 7, 2016 after a ConAgra employee observed metal fragments while dispensing sugar from a supplier for a P.F. Chang’s Home Menu Brand sauce ingredient. On July 14, 2016, ConAgra Foods was notified by the supplier of additional production lots of sugar that were impacted, such that the initial recall needed to be expanded to include additional P.F. Chang’s Home Menu Brand meals. Products covered by this recall were distributed in the U.S., specific product names and information are listed here. No other ConAgra Foods are impacted by this recall and these products are not served in P.F. Chang’s restaurants. Consumers who have purchased these items are advised not to consume them and return them to the store where originally purchased. Recall Date: 07/16/2016
  • Agave Dream Cappuccino Ice Cream: Agave Dream is recalling 389 cases Cappuccino Ice Cream because it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Recalled product is Agave Dream Cappuccino ice cream pints packed in a brown paper pint size container with “Agave Dream” printed front of carton with Best By date 07/04/17, UPC 899349002048. Product was distributed across the United States through retail stores receiving product from KeHE Romeoville, Illinois warehouse, DPI NW, Americold L.A., IWI Franklin Indiana warehouse, Haddon House Richburg, SC. Consumers who have purchased Agave Dream Cappuccino ice cream with “Best By” date 07/04/17 are urged to return it to the place of purchase for a full refund. Recall Date: 07/19/2016
  • Siluriformes Fish Products: Haring Catfish, Inc. is recalling approximately 21,521 pounds of siluriformes fish (catfish) products that may be adulterated with a residue of public health concern, specifically gentian (crystal) violet, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The siluriformes (catfish) products items were produced on June 28 and 29, 2016. Products included in the recall can be found here. The products subject to recall bear establishment number “EST. 51217” inside the USDA mark of inspection. These items were shipped to retail locations and hotels, restaurants, and institutions in Arkansas, California, Louisiana, Mississippi, Oklahoma, and Texas. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 07/14/2016
  • Chicken and Pork Hot Dog and Corn Dog Products: Bar-S Foods Company is recalling approximately 372,684 pounds of chicken and pork hot dog and corn dog products that may be adulterated with Listeria monocytogenes, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The ready-to-eat, chicken and pork hot dog and corn dog items were produced on July 10, 11, 12, and 13, 2016. Products included in the recall can be found here. The products subject to recall bear establishment number “EST. P-81A” inside the USDA mark of inspection. These items were shipped to retail locations nationwide. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 07/19/2016
  • Cannondale Commuter Bicycles: Cycling Sports Group, Inc. is recalling 2010-2012 Cannondale Bad Boy and Bad Girl commuter bicycles. The bicycle’s fork can crack and cause a fall hazard. They were sold in black. Only bicycles with date codes beginning with P, Q and RB through RL are included in this recall. The date code is located on the bottom of the bicycle fork. The bicycles were manufactured in Taiwan and sold at authorized Cannondale dealers nationwide from June 2010 through December 2014. Consumers should immediately stop using the recalled bicycles and take them to the nearest authorized Cannondale dealer for a free repair. Cannondale dealers will replace the fork free of charge.   • Crosman Crossbow Rope Cocking Devices: Crosman Corporation is recalling Rope cocking devices for Centerpoint Sniper 370 Crossbows because tabs on the hooks attaching the rope cocking device to the crossbow string can bind as the device is pulled back, causing the hooks to break and the rope to recoil, posing a risk of injury, including laceration. This recall involves rope cocking devices shipped by Crosman Corporation with Centerpoint Sniper 370 crossbows with serial numbers beginning with “015.” The serial number is printed on the right side of the crossbow above the trigger. The Centerpoint Sniper 370 crossbow has a camouflaged stock and measures 36.5 inches long. The rope cocking device is a rope with a black plastic “T” shaped handle at each end. A pair of black plastic tabbed hooks are connected to the rope. The rope cocking device measures 45 inches from handle to handle. The words “CENTERPOINT” and “SNIPER 370” are printed on the crossbow’s frame. The products were sold at Academy, Cabela’s and other stores nationwide and online at Amazon.com, Crosman.com and Sportsmansguide.com from December 2015 through June 2016. Consumers should immediately stop using the recalled rope cocking device and contact Crosman to receive instructions on returning it for a replacement rope cocking device without tabs. Crosman has contacted all known purchasers. Recall Date: 07/21/2016
  • Lorex Video Baby Monitors: Lorex Technology, Inc. is recalling video baby monitors because The video monitor’s batteries can overheat, swell and expand and cause the battery cover to open or come off. This can expose hot batteries, posing a burn hazard to consumers. This recall involves Lorex Care ‘N’ Share video baby monitors. The model numbers included in this recall are WL3520, WL4320 and WL3401. The model numbers are printed on the back panel of the monitor. The monitors were sold in bundles with cameras. The monitors contain a blue lithium polymer battery and measure about 4 inches tall by 5 inches wide. The monitors have a white plastic back and either a white or black border. “LOREX” or “The Lorex Baby” is printed below the monitor screen. Babies “R” Us, B&H, Best Buy, Target, The Home Depot, Walmart and online at www.amazon.com and www.lorextechnology.com from May 2013 through April 2016. Consumers should immediately stop using the recalled monitors and contact Lorex to receive a full refund. Recall Date: 07/21/2016
  • Nu Skin Mist Diffusers: Nu Skin Enterprises, Inc. is recalling mist diffusers because mold can develop on the product, posing a health risk to individuals with compromised immune systems, damaged lungs or an allergy to mold. The diffuser is a plastic bowl with a glass lid and bamboo trim ring, used to diffuse essential oils. “Nu Skin” is engraved in the bamboo on the side of the product. The white and tan diffusers are 6.5 inches in diameter and 3.5 inches in height. The recalled lot numbers are PZ11351, PZ17051, PZ21551, PZ03151 and PZ03451. The lot number is printed on the white plastic on the bottom of the product. The product was sold exclusively online by nuskin.com from January 2015 through March 2016. Consumers should immediately stop using the recalled mist diffusers and contact Nu Skin for instructions to inspect the product for mold and for a free replacement mist diffuser if mold is identified. Nu Skin is contacting purchasers of the product directly. Recall Date: 07/21/2016
  • Pork Products: Kapowsin Meats, Inc. is recalling approximately 11,658 pounds of pork products that may be contaminated withSalmonella I, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. FSIS was notified of an illness investigation in Washington on July 13, 2016. The Washington State Department of Health updated FSIS on July 19, 2016 of confirmed case-patients involved in an illness outbreak of Salmonella  Based on epidemiological investigation, three Salmonella I case-patients have been identified with illness onset dates ranging from July 5, 2016, to July 7, 2016. Traceback investigation indicated that three case-patients consumed whole hog roasters for barbeque from Kapowsin Meats. The whole roaster hogs were produced between June 13, 2016 and July 15, 2016. The following products are subject to recall: varying weights of boxed/bagged Whole Hogs for Barbeque. The products subject to recall bear establishment number “EST. 1628M” inside the USDA mark of inspection. These items were shipped to various individuals, retail locations, institutions, and distributors in Washington. onsumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 07/19/2016
  • Honey Roasted Peanuts: Krispak, Inc. is recalling a small quantity of Honey Roasted Peanuts packed under the GFS label because they may contain undeclared Tree Nuts – Pecans. People who have allergies to Tree Nuts – Pecans run the risk of serious or life-threatening allergic reaction if they consume this product. The recalled GFS Honey Roasted Peanuts were distributed to Gordon Food Service distribution centers in FL., MA., MI., OH and PA. The product comes in a 38 oz PET (Polyethylene terephthalate) plastic jar marked with lot number 176-16 stamped above the UPC code 093901252562 and GFS reorder number. Consumers who have purchased GFS HONEY ROASTED PEANUTS with the above stated code dates are urged to return them to the place of purchase for a full refund. Recall Date: 07/20/2016
  • Hy-Vee No-Salt Added Black Beans: Hy-Vee, Inc. is voluntarily recalling a limited quantity of Hy-Vee No-Salt-Added Black Beans across its eight-state region due the potential presence of plastic and/or metal pieces within the product. The presence of small pieces of plastic and/or metal has the potential to cause a choking hazard and may cause adverse health consequences, including injury. The voluntary recall is limited to 15-ounce cans of Hy-Vee No-Salt-Added Black Beans with the code “Best By 04 APR 2019 258F2 095 16 XXXX” and UPC number of 0075450105190. The product was sold at Hy-Vee stores in Illinois, Iowa, Kansas, Minnesota, Missouri, Nebraska, South Dakota and Wisconsin between April 22, 2016, and July 18, 2016. The product was manufactured at the company’s Faribault, Minnesota, facility, which supplies several major retailers across the United States. Faribault Foods, Inc. has determined this to be an isolated incident. Out of an abundance of caution, Hy-Vee is recalling the product from all of its stores. No other Hy-Vee food items are impacted by this recall. Customers who purchased this product should discard it or return it to their local Hy-Vee store for a full refund. Recall Date: 07/20/2016
  • Peony Mark Bran Biscuits: Starway Incorporated is recalling Peony Mark Brand Biscuits because the product contains undeclared milk and unallowed color Ponceau 4R (E124). People who have severe sensitivity to milk run the risk of serious or life threatening allergic reaction if they consume this product. Peony Mark Brand Biscuits, a product of China, is distributed in uncoded and heat-sealed 6 oz. plastic bags. The product was distributed nationwide. The recall was initiated after routine sampling by NYS Dept. of Agriculture & Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the presence of undeclared milk and unallowed color Ponceau 4R (E124) which were not declared on the label. Consumers who have purchased Peony Mark Brand Biscuits should return it to the place of purchase for a full refund. Recall Date: 07/20/2016
  • HCG and Sermorelin: Talon Compounding Pharmacy (TCP) is voluntarily recalling all lots of lyophilized HCG and Sermorelin aseptically compounded and packaged by TCP and that remain within expiry due to the Food and Drug Administration’s (FDA) concern over a lack of sterility assurance. The sterile products were distributed to patients and providers nationwide between January 18 and July 18, 2016. The recalled lots were packaged in 10ml amber glass vials bearing a label containing the name and strength of the drug, the lot number and the beyond-use date. TCP is asking all patients and providers that received lyophilized HCG and sermorelin compounded products from TCP between January 18 and July 18, 2016, and that remain within expiry, to discontinue use of the products; quarantine any unused product until further instructions are received on how to return the product; and contact TCP.
