Product Recalls for the Week of May 8, 2017:

  • Herbalife Protein Bar: Herbalife is voluntarily recalling certain lots of its Protein Bar – Peanut Butter because the product may contain a trace amount of fish gelatin, which was added to the product as part of a flavoring ingredient provided by a third-party supplier. The amount of gelatin in the finished product is extremely low (approximately 0.02%). The gelatin is a highly-processed collagen from fish skin, which contains a negligible amount of parvalbumin, a fish allergen. The product was distributed in the U.S. from 6/30/2016 to 4/7/2017, exclusively to individual independent distributors as 14-pack cartons. Lots numbers affected are 066539A11, 066553A11, 066601A11, 066660A11, 066688A11, and 066691A11. Independent distributors and their customers that have purchased this product are urged to return the product for exchange or refund. Recall Date: 5/1/2017
  • Ben & Jerry’s Chocolate Fudge Brownie Pint Slices: Unilever is voluntarily recalling a limited number of boxes of Ben & Jerry’s Chocolate Fudge Brownie Pint Slices, which may inadvertently contain Vanilla Peanut Butter Cup Pint Slices. Although the slices were individually wrapped and identified as Vanilla Peanut Butter Cup Pint Slices, the ingredient peanut butter (containing the known allergen peanut), is undeclared on the outer product packaging. Persons who have an allergy or severe sensitivity to peanuts run the risk of a serious or life-threatening allergic reaction if they consume the product. The affected product is sold in a paperboard box, containing 9 fL oz (266 mL) (3 individually wrapped ice cream slices) with UPC code of 076840657940 best by date AUG1218LT2 and lot number of AUG1218LT2. The product was manufactured in the United States. The product was distributed nationwide and reached consumers through retail stores. Consumers who have purchased boxes of the above product with the affected UPC code and date code are asked to immediately discontinue use of the product and retain the outer box.   • Lithium Batteries for Motorized Window Coverings: Springs Window Fashions LLC and Great Power Battery (Zhuhai) Co. Ltd. are recalling motorized window coverings with lithium batteries because the lithium batteries that power the motorized window coverings can overheat, leak or discharge, posing fire and burn hazards. This recall involves custom motorized window coverings that were sold with Zeus brand AA lithium batteries. Window coverings with batteries included in this recall were sold under the Bali, Graber, and Signature Series brands, and as private labeled products of Blinds Galore and JCP Home. The custom window coverings include cellular, roller, Roman (fabric and natural shades), pleated, sheer and layered shades, and wood blinds. Only Zeus brand AA lithium batteries are included in this recall. Recalled products were sold at Budget Blinds, Gotcha Covered, Home Depot, Lowe’s, JC Penney, Menards and independent Graber dealers nationwide, and online at Blinds.com from December 2015 through November 2016. Consumers should immediately stop using and remove the Zeus brand batteries from the window coverings. Springs Window Fashions will ship batteries and instructions directly to consumers.   • Quality Nut Co. Assorted Nuts: Quality Nut Co. of Massillon, Ohio is voluntarily recalling the following products due to undeclared allergens: SWEET AND SPICY MIX – 2 oz. bags. Labels did not declare the allergens soy and wheat; SWEET AND SPICY MIX – 3 oz. bags. Labels did not declare the allergen soy; HUNTER TRAIL MIX – 2.5 oz. bags. Labels did not declare the allergen milk; CHOCOLATE PEANUTS – 5 oz. bags. Labels did not declare the allergen soy; and MILK CHOCOLATE PEANUTS – 4 oz. bags. Labels did not declare the allergen soy. Products were available for sale prior to May 1, 2017, at locations throughout Ohio. This issue was discovered during a routine inspection conducted by the Ohio Department of Agriculture. There have been no reports of illness involving products addressed in this recall, however, people who have an allergy or severe sensitivity to these allergens are at risk for a serious or life-threatening allergic reaction if they consume these products. Customers with allergies or sensitivity to these undeclared ingredients who purchased the affected products should dispose of it or return it to Quality Nut Co. for replacement. Recall Date: 5/5/2017
  • Aunt Jemima Frozen Pancakes, Frozen Waffles & Frozen French Toast Slices: Pinnacle Foods Inc. is voluntarily recalling all “Best By” dates of Aunt Jemima Frozen Pancakes, Frozen Waffles & Frozen French Toast Slices distributed nationally in the United States and one product into Mexico because they have the potential to be contaminated with Listeria monocytogenes. Pinnacle Foods Inc. is voluntarily recalling all “Best By” dates of Aunt Jemima Frozen Pancakes, Frozen Waffles & Frozen French Toast Slices distributed nationally in the United States and one product into Mexico because they have the potential to be contaminated with Listeria monocytogenes.   • GEC Laxoplex Dietary Supplement: Genetic Edge Compounds is voluntarily recalling all lot codes distributed between February 2, 2015- May 2, 2017, of GEC Laxoplex dietary supplement capsules, packaged in a white plastic bottle containing 60 capsules to the retail level and consumer level. Food and Drug Administration (“FDA”) analysis has found GEC Laxoplex to be tainted with anabolic steroids and steroid-like substances. The presence of these anabolic steroids and steroid-like substances in GEC Laxoplex renders it an unapproved drug for which safety and efficacy have not been established and therefore subject to recall. Use or consumption of products containing anabolic steroids may cause acute liver injury, which is known to be a possible harmful effect of using steroid-containing products.  In addition, abuse of anabolic steroids may cause other serious long-term adverse health consequences in men, women, and children. These include shrinkage of the testes and male infertility, masculinization of women, breast enlargement in males, short stature in children, a higher predilection to misuse other drugs and alcohol, adverse effects on blood lipid levels, and increased risk of heart attack, stroke, and death. GEC Laxoplex is marketed as a dietary supplement and sold as a muscle-enhancing agent.  The product is packaged in a white plastic bottle containing 60 capsules with UPC code 0058049984 and can be identified by GEC Laxoplex.  The recall affects all lots of GEC Laxoplex.  GEC Laxoplex was distributed Nationwide in the USA through various nutritional supplement retail outlets. Consumers and retailers that have GEC Laxoplex dietary supplement capsules which are being recalled should stop using them and return to place of purchase. Recall Date: 5/5/2017
  • Smallbatch Chicken Blend for Dogs and Cats: Smallbatch Pets Inc. is voluntarily recalling two lots of frozen 2lbs. chicken blend for dogs and cats, brand name Smallbatch, due to the potential to be contaminated with Salmonella. Salmonella can affect animals eating the products and there is a risk to humans from handling contaminated pet products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products. Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans. If your pet has consumed the recalled product and has these symptoms, please contact your veterinarian. The potentially affected lots of 2lbs. chicken blend was distributed to retail pet food stores in States CA, CO, OR, WA through pet food retailers/distributors.  Two hundred and eighty-two cases of this product were sold between the dates of 2/1/17 – 5/5/17. The affected products are sold frozen in 2lbs. bags.  The products affected by this recall are identified with the following manufacturing codes and the “Best By” date is located on the back of the package. Products include Lot no. D032, UPC 705105970974, Best By Date 2/1/2018; and Lot no. E058, UPC 705105970974, Best By Date 2/27/2018. Consumers who have purchased the above lots of Chicken Blend are urged to stop feeding them to their dogs or cats and return the product to place of purchase for a full refund or dispose of them immediately.   • Italian Style Organic Chicken Sausage: Perdue Foods LLC is recalling approximately 2,148 pounds of chicken sausage products that may be contaminated with extraneous materials, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The Italian chicken sausage links were produced on March 27, 2017. Products subject to recall include 24-ounce plastic packages containing eight links of fully cooked ready to eat “PERDUE HARVESTLAND Italian Style Organic Chicken Sausage” bearing package code 64405 and a sell or freeze by date of 6/25/17. The products subject to recall bear establishment number “P-2617” inside the USDA mark of inspection. These items were shipped to a retail distributor in Connecticut and Maryland. The problem was discovered on May 5, 2017, when FSIS was notified by the firm that they had received three consumer complaints of blue plastic materials in the sausage. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 5/6/2017
  • Amitriptyline HCL Tablets and Phenobarbital Tablets: Bellmawr, New Jersey, C.O. Truxton, Inc. is expanding their – 04/21/2017 voluntary recall, as a precaution to include the following C.O. Truxton, Inc. products, registered NDC numbers and corresponding lot numbers, to the consumer/user level. C.O. Truxton has not received any complaints about Amitriptyline HCL and Phenobarbital tablets – however, due to the initial recall resulting from a label mix-up error, out of an abundance of caution, we are recalling all products that were repackaged into a Truxton Incorporated label. If mislabeled, inadvertent exposure to, or overdose of phenobarbital could cause severe intoxication which may lead to cardiogenic shock, renal failure, coma, or death in humans and animals. If mislabeled, inadvertent exposure to, or overdose of amitriptyline could cause uneven heartbeats, extreme drowsiness, confusion, agitation, vomiting, hallucinations, hot or cold sensations, muscle stiffness, seizures (convulsions), or fainting in humans and animals. C.O. Truxton, Inc. has not received any reports of adverse events related to this recall. A complete list of recalled products can be found here.   • Mineo & Sapio Beef Patties: Ford Brothers Wholesale Meats, Inc. is recalling approximately 4,015 pounds of beef patties due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The product contains soy, a known allergen, which is not declared on the product label. The frozen raw beef patties were produced on August 26, 2016 through May 4, 2017. Products subject to recall include 5-pound boxes containing four patties per pound of “MINEO & SAPIO BEEF PATTIES,” 5-pound boxes containing six patties per pound of “MINEO & SAPIO BEEF PATTIES,” 5-pound boxes containing eight patties per pound of “MINEO & SAPIO BEEF PATTIES,” and 5-pound boxes containing ten patties per pound of “MINEO & SAPIO BEEF PATTIES.” The products subject to recall bear establishment number “EST. 4625” inside the USDA mark of inspection. These items were shipped to wholesale locations in New York. The problem was discovered on May 8, 2017 by FSIS personnel while conducting label review verification activities. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 5/8/2017
  • Publix Artichoke and Spinach Dip: Publix Super Markets is issuing a voluntary recall for Publix Deli Artichoke and Spinach Dip due to the possibility of the product containing small glass fragments. The 16 oz. artichoke and spinach dip was sold at Publix stores in Florida, Georgia, South Carolina, North Carolina, Alabama and Tennessee with a UPC of 000-41415-15961, and a use-by-date of  May 16 A1 and May 16 C1, which is printed on the lid of the container. The product in question is found in the refrigerated cases in the deli. Consumers who have purchased the product in question may return the product to their local store for a full refund. Recall Date: 5/8/2017
  • Recreational Off-Highway Vehicles: American Honda Motor Company Inc. is recalling its recreational off-highway vehicles (ROVs) because the front passenger seat belt buckle can fail due to a manufacturing defect, posing a risk of injury. This recall involves model year 2017 Honda Pioneer 700 side-by-side recreational off-highway vehicles. The ROVs are motorized with a steering wheel, a gas pedal, a brake pedal, bucket or bench seats, seat belts and an occupant protection structure. The recalled ROVs were sold in two-seat and four-seat models in blue, red, silver or camouflage colors. “HONDA” is printed on the front grill and rear tailgate. “Pioneer 700” is printed on the sides of the vehicle. The model and serial numbers are printed on a certification label located at the top rear of the left front wheel well. A complete list of model numbers and serial numbers can be found here. Recalled vehicles were sold at Authorized Honda Powersports dealers nationwide from December 2016 through April 2017. Consumers should immediately stop using the recalled ROVs and contact their local authorized Honda Powersports dealer to schedule an appointment for a free inspection and repair.   • Save-A-Lot Pick 5 Mix & Match Chicken & Waffle Sandwich: Fourth Street Barbecue Inc. Packing Division has initiated a voluntary recall of all lot numbers of the Save-A-Lot Pick 5 Chicken Waffle Sandwich distributed nationwide as it has the potential to be contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. The product was sold frozen and distributed nationwide to Save-A-Lot stores. Products subject to recall include 14.4 ounces/box Save-A-Lot Pick 5 Mix & Match Chicken & Waffle Sandwich, UPC 051933353664. Consumers that have any of these products in their possession are asked not to consume the products. Recall Date: 5/9/2017
  • Polaris Snowmobiles: Polaris Industries Inc. is recalling its RMK and SKS snowmobiles because a weld on the steering post can separate, resulting in loss of steering control, posing a crash hazard. This recall involves model year 2017 Polaris RMK, PRO-RMK, SKS, and RMK snowmobiles. “Polaris” is printed on the seat and “RMK,” “PRO,” “SKS,” or “ASSAULT” printed on the side body panel. The snowmobiles were sold in several colors. The model number and VIN are displayed on the right side of the tunnel. To check for recalled vehicles by VIN, visit www.polaris.com.   • Bicycle Handlebar Stems: Profile Design LLC is recalling its Cobra S handlebar stems because the bicycle handlebar stems can corrode and break, causing the rider to lose control and crash. This recall involves Profile Design Cobra S carbon-wrapped black handlebar stems sold individually and as original equipment on the following Kestrel, Fuji, Jamis and Scott bicycle models: 2009  Kestrel  Talon SL Tri Ultegra, 2009  Kestrel  Evoke SL, 2010  Kestrel  Talon SL Tri- Ultegra, 2010  Kestrel  Talon Tri- Force, Fuji 2009 D-6 Pro, Fuji 2010 D-6 Pro, Fuji 2010 D-6 Matt Reed, Jamis 2010 Xenith T2, Scott 2008 CR1 Plasma LTD (model# 209562), Scott 2009 Plasma Premium (model # 212052) and Scott 2010 Plasma Premium (model # 215722). The stems were sold in seven lengths or sizes including 60 mm, 70 mm, 80 mm, 90 mm, 100 mm, 110 mm and 120 mm. “Profile Design” and “Cobra S” are printed in white on the black handlebar stems. Only black stems are included in this recall. Recalled stems were sold at independent bicycle stores nationwide from January 2007 through December 2013. Consumers should immediately stop using bicycles with the recalled handlebar stems and contact Profile Design for instructions to receive a free replacement stem. Recall Date: 5/9/2017
  • Wegmans Potato Salad: A.S.K. Foods, Inc. is voluntarily recalling one lot of Wegmans Family Pack Food You Feel Good About Net Wt. 48 oz. (3 LB.) 1.46 KG Potato Salad because it may contain undeclared egg.  People who have an allergy or severe sensitivity to egg run the risk of serious or life-threatening allergic reaction if they consume these products. The recalled potato salad was distributed and sold at Wegmans Food Markets in NY, MA and Erie, PA. The recalled product was packaged in 48-ounce (3 Lb.) clear plastic containers having a label that states Wegmans Family Pack Food You Feel Good About Potato Salad on the lid. The recalled product has a code on the lid that states “SELL BY: 5/7/17 3379-4” and is marked with UPC 77890 24678 on the side label. The recall was initiated after it was discovered that certain packages of the recalled product contained an ingredient and nutrition label from another product [Greek Pasta Feta Salad] that did not reveal the presence of egg.  People who have an allergy or sensitivity to eggs should not consume the potato salad.  If you still have the salad, please return it to the Wegmans store that you purchased it from for a full refund.   • Monte Cristo Turkey Breast, Ham and Cheese Sandwiches: Kabob’s Acquisition, Inc. is recalling approximately 5,946 pounds of turkey and ham sandwich products which included a non-meat ingredient that was recalled due to Listeria monocytogenes, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The Monte Cristo (turkey, ham, cheese, on French toast) sandwiches were produced from May 1, 2014 through February 1, 2017. Products subject to recall include 13.25-pound bulk cases containing 200 “Kabobs MONTE CRISTO TURKEY BREAST, HAM AND CHEESE SANDWICH” in shrink wrapped plastic trays and bearing case codes: PM14E27, PM14H14, PM14J10, PM14E01, PM14E12, PM14E13, PM15C26, PM15E06, PM15G23, PM14L18, PM15A08, PM15C26, PM15K20, PM16B08, PM16B09, PM16F22, PM16F24, and PM17B01. The products subject to recall bear establishment number “P-6640” inside the USDA mark of inspection. These items were shipped to institutional locations in Florida, Georgia, South Carolina, and Texas. Recall Date: 5/10/2017
  • Night Lights: AM Conservation Group Inc. is recalling its luminescent night lights because the night lights can overheat, posing a fire hazard. This recall involves luminescent night lights Model 2017-G, a square-shaped night light with a rounded top. The night light glows green when plugged into an electrical outlet. The back of the night light has a metallic sticker with the “UL” logo on it. If the UL label has a number on it beginning with the letter H or I followed by seven numbers, it is included in this recall. The night lights were given free to consumers, individually and in energy conservation kits. Recalled night lights were distributed as free promotional products by various companies between November 2016 and March 2017. Consumers should immediately stop using the recalled night lights and contact the firm for a free replacement. Recall Date: 5/10/2017
  • Mikawaya Brand Chocolate Chocolate Mochi Ice Cream: Mikawaya is issuing a voluntary recall of a limited number of Mikawaya Brand Chocolate Chocolate Mochi Ice Cream, which may inadvertently contain peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of a serious or life-threatening allergic reaction if they consume the product. Only product labeled with UPC code of 070934990609 and lot number LOT 090-17 is included in this recall. The UPC code can be found on the back of the box and the lot number is printed on the top enclosure. The affected product is sold in a paperboard box, containing 9.1oz (258g) of 6-1.5oz (43g) pieces with UPC code of 070934990609 and lot number LOT 090-17. The product was distributed to all Trader Joe’s stores nationwide and has been removed from sale. Customers who have purchased Mikawaya Chocolate Chocolate Mochi Ice Cream with the affected UPC code and lot number, and have an allergy or sensitivity to peanuts, are advised not to consume the product. Customers may return the product to any Trader Joe’s for a full refund. Recall Date: 5/10/2017
  • Multi-Grain Alaskan Cod: Trident Seafoods Corporation is issuing a voluntary recall of select lots of frozen Multi-Grain Alaskan Cod, Net Wt. 12, oz., because they may contain small pieces of plastic. Consumption of these products may pose a choking hazard orother physical injuriesy to the mouth. Retail product subject to the voluntary recall includes Trident Seafoods Multi-Grain Alaskan Cod (Frozen), Net Wt. 12oz., UPC 0 28029 21048 4. Recalled Lot Numbers with associated Best By dates are printed on one end of the individual retail cartons and on the case label: Lot Number A633511, Best By: 11/30/2018; and Lot Number A636225, Best By: 12/27/2018. Lot Number A633511 was distributed between 12/1/2016 and 2/9/2017 to Arizona, California, Colorado, Illinois, Kansas, Massachusetts, Minnesota, New Jersey, Pennsylvania, Texas, Washington, and Wisconsin. Lot Number A636225 was distributed between 1/23/2017 and 2/17/2017 to Arizona, California, Colorado, Illinois, Kansas, Massachusetts, Texas, Washington, and Wisconsin. These products are sold at Albertsons, Amazon, Cub Foods, Jewel, Morey’s, Plaza Extra, Shaw’s, Shoprite, Sprouts, SUPERVALU, and Woodman’s retailers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 5/11/2017
  • Red Robin Burgers and Brews Crispy Onion Rings: Lamb Weston is recalling 14 ounce packages of Red Robin Burgers and Brews Crispy Onion Rings manufactured by Fry Foods and sold in grocery stores, because the product may contain an undeclared milk allergen. People who have an allergy or severe sensitivity to milk run the risk of serious or life-threatening allergic reaction if they consume this product. These products are NOT served in Red Robin restaurants. The Red Robin Burgers and Brews Crispy Onion Rings were distributed in the U.S. through retail grocery stores. No other products are impacted by this recall. The product comes in a 14 ounce bag and is frozen. Details on the impacted packages are listed below. The Sell By Date and Package Code can be found on the back of the package. The HH:MM portion of the Package Code indicates the time product was made and will vary by package. Products subject to recall include Red Robin Crispy Onion Rings, UPC 0 43301 61119 3, Net Weight 14 ounces, sell by OCT 28 2018 and OCT 29 2018, Package Code 6906711841 HH:MM and 6906711941 HH:MM. Consumers who have purchased Red Robin Burgers and Brews Crispy Onion Rings from a grocery retailer are urged to return the product to the place of purchase for a full refund. Recall Date: 5/11/2017
  • Heat Pumps: Carrier Corporation is recalling its Carrier Greenspeed™ and Bryant Evolution Extreme™ Heat Pumps because the capacitors in the fuse boards in the heat pumps can stop working causing the unit to overheat, posing a fire hazard. This recall involves 2, 3, 4, and 5 ton size heat pump units sold under the Carrier Greenspeed and Bryant Evolution Extreme brand names. The units are used for cooling and heating homes. The Carrier Greenspeed model numbers are: 25VNA024, 25VNA036, 25VNA048, and 25VNA060.  The Bryant Evolution Extreme model numbers are:  280ANV024, 280ANV036, 280ANV048, and 280ANV060.  The model number can be found on the unit nameplate (or rating plate) located on one side of the unit’s exterior. On the Bryant unit, there is a label on top of the unit that reads “Bryant Evolution System.” Recalled units were sold at Sears stores and HVAC dealers nationwide from June 2011 through August 2016. Consumers should contact Carrier or Bryant for instructions on receiving a free replacement fuse board installed by authorized Carrier or Bryant technicians. Recall Date: 5/11/2017
  • Nature’s Truth Iron Supplement: Nature’s Truth LLC is recalling Nature’s Truth Slow Release Iron Supplements because the packaging is not child-resistant as required by the Poison Prevention Packaging Act. The tablets inside the bottle contain iron, which can cause serious injury or death to young children if multiple tablets are ingested at once. This recall involves Nature’s Truth Slow Release 45mg Iron Supplement bottles. Recalled supplements were sold at Quick Chek stores in New Jersey and independent pharmacies nationwide from February 2017 to March 2017.   • Poultry Products: NY Livestock Market, Inc. is recalling approximately 1,495 pounds of poultry products that may be contaminated with the antibiotic nitrofurazone, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced. The whole young chickens were produced and packaged on April 10, 2017. Products subject to recall include brown shipping bags containing various quantities of 4.9-lb. young whole chickens, with head and feet intact, with “NY LIVESTOCK MARKET, INC. BUDDHIST RELIGIOUS EXEMPT (Eviscerated Poultry)” printed on the label. The products subject to recall bear establishment number “P-45068” inside the USDA mark of inspection. These items were shipped to restaurants and retail locations in New York state. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase. Recall Date: 5/12/2017
